  1. Print the Customer Claim forms [Motor Vehicle Fuel Service Report] and [Service Data Report] on your printer or call 608-781-8988; ask for Petroleum and we will send you the proper forms. Quality Gasoline Guaranteed!
  2. Fill out completely the Customer Data portion of the form.
  3. Have your mechanic or repair facility fill out the Service Data portion of the form.
  4. Send the completed form along with copies of your repair bill to the address on the form.
  5. We will reimburse you for any legitimate fuel-related repairs and the gasoline.
